A clear difference exists in care management strategies between high performing and low performing clinical practices. Evaluating successful approaches in busy practices can give us a path for success.

Incorporation of performance data to support pre-visits and post-visit assessments aid in the delivery of high quality care. But having a reporting structure is not enough, and requires devoting time to evaluate the data.
