  • Cloud based real time patient data analytics & predictive modeling

  • Facilitate earlier detection of disease exacerbation

  • Timely optimized personalized interventions tailored to individual patient needs

  • Remote monitoring enhancing patient engagement

  • Decision support tools

  • Collaboration among stakeholders
